This survey is about the Health and Health Care of older Adults in the Finger Lakes Region. It is very important to the work that the SAGE commission is doing for the future planning of our aging population.

Finger Lakes Health Systems Agency is a not-for-profit organization responsible for planning health systems that insure that all people in the nine county area have simple access to the right care, at the right time, at a reasonable cost.

The SAGE Commission has been established to create a vision and design the services needed for those 65 and over by 2020 and beyond. We are asking you to take a few minutes to fill out a brief survey to help the planning commission prioritize factors that will shape the future services for older adults in the Finger Lakes Region. We are also asking that you encourage others who are concerned about the health and health care of the aging to complete the survey as well.

As the survey responses will be used for the SAGE Strategic Visioning meeting later this month, we ask that you please complete the survey by June 20th.
